广州公信招标有限公司受招标人委托对下列产品及服务进行国际公开竞争性招标，于2020年5月6日在中国国际招标网公告。本次招标采用传统招标方式，现邀请合格投标人参加投标。

3.投标人资格要求
投标人应具备的资格或业绩：
3.1 投标人经合法注册。（The Bidder shall be legally registered.）
3.2 投标人在中国国际招标网（www.chinabidding.com）成功注册。（The Bidder shall have registered on www.chinabidding.com.）
（注：投标人在招标文件要求的投标截止时间前应当在招标网在线填写招投标注册登记表，并将由投标人加盖公章的招投标注册登记表及工商营业执照（复印件）提交至招标网；境外投标人提交所在地登记证明材料（复印件），投标人无印章的，提交由单位负责人签字的招投标注册登记表。投标截止时间前，投标人未在招标网完成注册的不得参加投标，有特殊原因的除外。投标人办理注册和年检事宜，可拨打网站服务咨询电话：400-0606-000。） （Note: The Bidder shall fill in the Registration Form on www.chinabidding.com before the bid deadline stipulated by the bid document, and shall submit the Registration Form sealed by the Bidder and the Business License (copy) to the above-mentioned website; Overseas bidders shall submit evidentiary materials of local registration (copy). 3.3投标人必须具有食品药品监督管理部门颁发的有效的《医疗器械生产企业许可证》或《医疗器械经营企业许可证》，及食品药品监督管理部门颁发的有效的《中华人民共和国医疗器械注册证》。（The bidder must have issued by the food and drug supervision and management departments of “medical device production enterprise license or the medical device business license”, and food and drug supervision and administration department issued the effective of the people’s Republic of China Medical equipment registration certificate.）
3.4 投标人必须提供由设备制造商或授权的中国总代理签署的合法有效的正式授权书或代理证明。若所投产品为国产产品，则无需提供。（The bidder must provide a legal and valid formal power of attorney or agency certificate signed by the equipment manufacturer or an authorized Chinese general agent. If the investment is a domestic product, it does not need to be provided.）
3.5 是否接受联合体投标：不支持。（Not acceptable.）
3.6 未领购招标文件是否可以参加投标：不可以。（Not allowed.）
4.招标文件的获取

4.4招标文件售价：￥300元或＄50（RMB 300 or USD 50）
其他说明：招标文件售价为每份人民币300元或50美元，售后不退，如需邮寄，国内另加60元人民币或10美元，国外另加200元人民币或30美元。（The price of each bid document is RMB 300 or USD 50, and refund is not applicable. If mailing is required, an extra RMB 60 or USD 10 shall be charged in the case of domestic mails, and RMB 200 or USD 30 in the case of international mails.）
4.5 其他补充说明：供应商凭以下资料加盖单位公章购买招标文件：（The Suppliers shall purchase the bid document upon their presentation of the following documents which shall have been sealed）
4.5.1 企业法人营业执照（副本）复印件或经合法登记证明材料复印件；（A copy of Business License (counterpart) or a copy of evidentiary materials of legal registration;）

4.5.2.1 经办人如是单位负责人，需提供单位负责人证明书及单位负责人身份证复印件；（In the case that the transactor is the person in charge of the company, the copy letter of statement and a copy of his ID card shall be required;）
